• Have underwent a prior treatment program
• Have been discharged or need ongoing care from the prior treatment program

Terms:

• Monthly recurring subscription
• Must be maintained for 12 consecutive months
• Cannot refund already charged purchase
• New complaints cannot be treated under a maintenance program - this would be an example of a covered service with a health insurance policy
• 2 Services per visit included
• Maximum of 2 visits per calendar week
• Only 1 person covered under each maintenance plan - no family plans available at this time
