• Your health condition must qualify - Acute care not included
• Have underwent a prior treatment program
• Have been discharged or need ongoing care from the prior treatment program

Terms:

• Monthly recurring subscription
• Must be maintained for 12 consecutive months
• Cannot refund already charged purchase
• New complaints cannot be treated under a maintenance program - this would be an example of a covered service with a health insurance policy
• 2 Services per visit included
• Maximum of 1-2 visits per calendar week based on chosen plan
• Only 1 person covered under each maintenance plan - no family plans available at this time
• No prorated refunds
• Plan will renew every year unless you notify us otherwise!

Defining Maintenance Treatment: Ongoing treatment for maximally improved complaints. Pain is still present with this type of care but is not expected to fully resolve

Defining Wellness Treatment: Treatment that continues when pain free to maintain status
